Effective August 14, 2026 · Applies to current and future Game Ready Baseball activities
PLEASE READ CAREFULLY. By registering a minor participant for Game Ready Baseball, I confirm that I am the participant’s parent or legal guardian, or that I otherwise have legal authority to give the permissions below.
1. Activities Covered
This agreement applies to the participant’s current and future participation in Game Ready Baseball activities, including camps, clinics, private or group lessons, development programs, workouts, conditioning, drills, games, events, and related activities, whether held at Irvine Valley College or another location, until I revoke this authorization in writing for future activities.
2. Assumption of Risk
I understand that baseball and athletic training involve inherent risks, including being struck by a baseball, bat, equipment, or another participant; collisions; falls; strains, sprains, fractures, concussions, and other injuries; weather and field conditions; use of training equipment and facilities; and other risks that may result in serious injury, illness, disability, or, in rare cases, death. I knowingly and voluntarily accept these risks on behalf of the participant.
3. Release of Liability
To the fullest extent permitted by California law, I release and hold harmless Game Ready Baseball and its owners, coaches, instructors, employees, independent contractors, volunteers, agents, and representatives from claims arising from the participant’s involvement in covered activities, including claims arising from ordinary negligence. This release does not apply to gross negligence, reckless misconduct, intentional misconduct, or any liability that cannot lawfully be waived.
4. Medical Condition & Emergency Care
I certify that the participant is physically able to take part and that I have disclosed any medical condition, allergy, injury, limitation, medication, or other information that Game Ready Baseball should reasonably know. If I cannot be reached in an emergency, I authorize Game Ready Baseball personnel to obtain reasonable emergency medical care for the participant. I understand that I am responsible for medical costs incurred on the participant’s behalf.
5. Rules, Conduct & Safety
I agree that the participant will follow reasonable safety instructions and program rules. Game Ready Baseball may remove a participant from an activity when reasonably necessary for safety, conduct, or disruption.
6. Facilities & Third Parties
I understand that Game Ready Baseball may use facilities owned or operated by third parties. Game Ready Baseball is an independent program and is not affiliated with Irvine Valley College unless expressly stated otherwise.
7. Continuing Agreement
This waiver is intended to remain in effect for the participant’s future Game Ready Baseball activities so that a new waiver is not required for each camp or lesson. I may revoke it for future participation by giving Game Ready Baseball written notice before the participant’s next activity. Revocation does not affect activities that occurred before notice was received.
8. Electronic Acceptance
By checking the waiver box and submitting the registration, I acknowledge that I have had the opportunity to read this agreement, understand it, and voluntarily agree to it as the participant’s parent or legal guardian. I understand that my electronic acceptance is intended to serve as my signature.
